The open enrollment period will be until January 30, 2012. You can enroll in the plan if you are eligible for Medicare Part A and/or you are enrolled in Medicare Part B and live in the service area.

Eligible persons may only enroll in one Medicare Part D Pharmacy Coverage at a time and may not be subscribed to a Medicare Advantage Plan (HMO, PPO) unless they are enrolled in a Medicare Fee-for-Service private plan that does not have pharmacy coverage or to a Cost 1876 Plan.

How can I renew the FarmaMed ELA Plan?

This year, your pharmacy contract will be automatically renewed with the new benefits 2012. You will receive your new card that will allow you to receive your new benefits starting January 1, 2012. This means you do not have to complete any form to process your renewal.

This renewal applies only to your Triple-S FarmaMed ELA plan. Your Medicare Supplemental Policy or Regular Plan with basic coverage will remain in the extension period until February 29, 2012, as established by the Health Insurance Administration (ASES).

If you want to make changes to your contract, the Health Insurance Administration (ASES) established a subscription period for changes from December 5, 2011 to January 30, 2012. The changes you make to your contract will be effective on March 1, 2012.

Remember to pay off any outstanding debts with Triple-S Salud to receive uninterrupted service.

Triple-S FarmaMed ELA is a prescription drug plan with a Medicare contract. The plan is available to all Medicare beneficiaries with Parts A, B or both that reside in the service area. Beneficiaries must use network pharmacies to access their prescription drug benefit, except in non-routine circumstances. Limitations and restrictions may apply. Benefits, formulary, pharmacy network, premium, copayments and/or coinsurances may change on January 1, 2013.
