Triple-S Salud offers educational services and health disease management to plan members through its Wellness Program. The Disease Management Program attends to some of the chronic conditions that most Puerto Ricans have: diabetes, hypertension and heart problems.
This program includes services such as clinical treatment (rendered by your physician); follow-up by professional nursing personnel and health education on how to care for each condition.
The Wellness Program has a team of nurses specially trained to provide orientation via telephone to plan members when the condition merits it or when plan members request it. In this case, a group of nurses and health educators will give continuous follow-up to your health needs, will instruct you on how to feel better, and will mail educational material to your home. They will also keep your physician posted on your health progress and will review with you the treatment guidelines for the conditions previously mentioned.
If the condition is not severe, the plan member can participate in educational activities, receive information bulletins via mail and participate in community activities such as health fairs and special clinics. This will help him/her avoid complications and improve his/her quality of life.
The educational program on the management of health conditions offers different workshops for each of the conditions included in the Integral Care Program:
Diabetes Program
Patient education program for plan members who are 18 years of age or older and who have diabetes on topics such as:
- What is diabetes?
- Emotional aspects of living with diabetes
- Exercise
- Nutrition
- Use of prescription drugs
- Complications of diabetes, among others
Hypertension Program
This program consists of educational activities focused on plan members who have hypertension (high or uncontrolled blood pressure) and are 18 years of age or older. You will learn:
- What is hypertension?
- What are the signals and symptoms
- How to modify your lifestyle
- How to keep your blood pressure under control
Heart Failure Program
This program is for plan members who are 18 years of age or older and who suffer from heart failure. Heart failure is a cardiac condition that occurs when a problem with the structure or function of the heart impairs its ability to pump enough blood to the rest of the body.
When the condition is severe, the nursing professionals of the Heart Failure Management Program will guide you about how to care for yourself so that you can feel better.
When you participate in the program, you will receive educational materials in your home. Patients whose condition is not severe will be invited by the health educators to educational activities. All this will help you to better control your condition, prevent complications and improve your quality of life.
