How can I enroll or renewal to Care Plus ELA?

Eligibility for Care Plus ELA:

If you are 65 years of age or older or are disabled and have both Medicare Parts A and B and live in our service area, Care Plus ELA is for you.

How can I enroll in Care Plus ELA?

To enroll in Care Plus ELA, you must:

• Complete the Enrollment form. Remember to complete all areas in the formulary including:
  • Name and both, first and second name of the member
  • Date of birth
  • Sex
  • Social Security
  • Phone Number
  • Residential Address
  • Postal Address
  • Evidence of the Parts A and Part B of Medicare
  • Beneficiary resides within the service area of the plan
  • Date of signature of the insured
  • Enrollment form must be signed by member or legal representative (requires evidence in writing)

Once completed in all its parts, can be delivered at any of our Service Centers or mail them to:

Remember that the enrollment and renewal period will be December 5, 2011 to January 30, 2012.
